Please complete the equipment donation form below, providing as much information as possible. We aim to respond to all equipment donation form submissions within 10 working days.

We are always grateful to receive offers of donations although because our UK premises are CQC registered, we are not able to accept the following:

  • Out-of-date consumables or medicines
  • Branded scrubs and uniforms
  • Any type of X-ray equipment
  • Donation offers from outside the UK

There is no need to fill in a form for each item you wish to donate, simply state a generic description such as “various hand instruments”.
